Since the beginning of aviation, human error has been recognized as a major factor in
accidents and incidents. Indeed, one of aviation’s biggest challenges has been, and will continue to be human error avoidance and control. Traditionally, human error in aviation has been closely related to
operational personnel, such as pilots, controllers, mechanics, dispatchers, etc.
